Health Forms

CRITICAL DEADLINE: JULY 24

All required student health forms must be submitted by July 24. Important Note: In accordance with school policy, students will NOT be permitted to attend school until all required forms are successfully submitted.

Submission Instructions

To ensure your child's forms are processed correctly, please note that different forms are sent to different departments:

• General Student Health Forms & Physicals: Turn in physically to the Main Office OR email as a PDF attachment to healthroom@damien.edu.

• Athlete Physical Forms: Must be uploaded directly to Healthy Roster (do not send these to the health room).

Please review the categories below to download and complete the forms necessary for your student:

1. Student Health Records

• Requirement: REQUIRED for all new students; physical updates are required annually for all returning students.

• Description: General health history, immunization records, and annual physical examination results.

2. Store-Administer Medication Form

• Requirement: OPTIONAL (Complete only as needed)

• Description: Must be filled out if your child requires the school health room to store or administer any prescription or over-the-counter medication during the school year.

3. Athlete Physical Form

• Requirement: OPTIONAL (Required only if your child is a prospective student-athlete)

• Description: Sports-specific physical clearance required for participation in school athletics.

• Reminder: These must be uploaded to Healthy Roster.
